All patients with a qualifying POAF event, who decline randomization, will be offered the option of enrollment in a parallel registry that captures their baseline risk profile and their treatment strategy in terms of anticoagulants or antiplatelets received. These patients will also be asked to fill out a brief decliner survey. The purpose of the research is to determine whether repairing a tricuspid valve (TV) in patients with mild to moderate tricuspid regurgitation (TR), at the time of planned mitral valve surgery (MVS), would improve the heart health of those who receive it compared to those who do not. Building a barbecue is a common DIY pursuit, and one that comes with a tasty payoff at completion. While many projects focus on charcoal or wood-fired designs, [Andrew] is more of a gas man. Not one to simply buy off the shelf, he designed his own burners from scratch. This quest wasn’t just unnecessary yak shaving; burners to suit [Andrew]’s desired size and power simply weren’t available. The burner is designed around the Venturi effect, wherein the propane gas is passed through a small orifice, creating a jet and pulling air along with it as it enters the burner tube. This causes the gases to mix, and they can then be ignited when passing through the outlet holes of the burner. Get the orifice and outlet holes sized just right, and you’ll have a burner that produces a hot, blue flame, perfect for efficient cooking. The orifice was produced with brass plumbing components, and hooked up to a valve rated for use with gas lines. The burner tube itself was created from stainless steel tube, with slots cut to act as outlet holes and with the end crimped and welded shut. [Update: On 10/23/19, the NSF committee responsible for revising NSF 61 tentatively agreed to tighten the limits on lead leaching from new faucets and drinking fountains. The committee will move forward with a formal vote and, if approved, will receive public comment on the proposed changes.] Last year, we discovered and reported in a blog, that some new brass faucets that meet existing standards and are labelled “lead-free” can still leach significant amounts of lead into water in the first few weeks of use. Here, we answer some questions that have come up and provide an update on efforts to revise the NSF/ANSI 61 standard to better protect and inform consumers. Last November, the committee responsible for revising the NSF/ANSI 61 standard convened a group to consider an optional certification for faucets that meet a more protective limit. A study of more than 500 models of faucets showed that 73% of faucets leach less lead into water and can meet a limit that is five times more protective for children. Sheila Briddick was left ‘in agony’ at home after contracting the deadly disease in her leg, forcing her local GP to request an ambulance to take her to hospital.  The 83-year-old was waiting for an ambulance but was first sent to small a crew, leading to a longer wait during which medics rated Mrs Briddick’s ‘pain level at 100′. Hours later she arrived at Darlington Memorial Hospital, where she waited for more than an hour for a bed before dying from the disease later that night.  During an inquest at Crook Civic Centre, coroner Oliver Longstaff recorded a verdict of death by natural causes. But he said it was ‘impossible to say’ whether she might have survived had she arrived at hospital, or been given antibiotics, sooner. I’ve ridden a bunch of lower power torque sensing ebikes from the likes of Bosch and Yamaha over the last several years. I’ve always found the power wanting, as most are designed for the Euro 250W market. Some of the newer torque-sensing electric mountain bikes brag about 750W peak power, but with only 450W continuous. Bafang has released a new drive unit called the M600 which is designed for the US 750 Watt continuous market and the drive can get 1000W peak. Lunacycle took that drive and shunted the controller effectively almost doubling the peak power to 2000W for their new X-1 bike. I plunked down a huge chunk of my own hardly earned cash when this bike first went on pre-sale almost 4 months ago because I knew it was the bike I had been waiting for Lunacycle to build. I got my first ride on it today, and it did not disappoint. The X-1 is probably the best ebike I have ever ridden, it feels like a downhill bike, but with the power of a high power ebike It’s hard for me to get bikes in my size as I am 6′ 9″ tall. Paul Jasa, Extension engineer at the University of Nebraska-Lincoln, describes strip-till as a transitional tool because it calls for tillage between strips, as opposed to no-till, which eliminates tillage altogether. When thinking long-term about your soil health, keep in mind that no-till farming increases resiliency best.
